Description

Virataz R is composed of Ritonavir (RTV) – a peptidomimetic inhibitor of the HIV-1 protease, and Atazanavir (ATV) – an azapeptide HIV-1 protease inhibitor (PI).

Indications:

- treatment of HIV-1 (Human Immunodeficiency Virus Type 1) infection in adult patients in case they do not have a history of virologic failure.

MOA (mechanism of action):

The principal action of Atazanavir and Ritonavir is to inhibit the virus-specific processing of viral Gag-Pol proteins in HIV-1 infected cells, thus averting formation of mature virions and infection of other cells. Ritonavir 100 mg is usually used as a pharmacokinetic booster of plasma levels of concomitantly administered protease inhibitors. Boosted protease inhibitors are currently considered the standard of care in HIV therapy.

Dosage

Concomitant therapy in treatment-naive patients

For treatment-naive patients: 1 tablet with food per day.

One Virataz R tablet once daily with food if combined with any of the following:

Tenofovir

H2-receptor antagonist (Famotidine 40 mg twice daily). Virataz R tablets should be administered concomitantly with, and/or at least 10 hours after, the dose of the H2-receptor antagonist.

Proton-pump inhibitors (Omeprazole 20 mg and must be taken approximately 12 hours prior to the Virataz R dose).

Concomitant therapy in treatment-experienced patients

For treatment-experienced patients: 1 tablet with food per day.

H2-receptor antagonist dose should not exceed a dose comparable to Famotidine 20 mg twice daily.

Virataz R tablets should be administered concomitantly with, and/or at least 10 hours after, the dose of the H2-receptor antagonist.

One Virataz R tablet once daily with food if taken with an H2-receptor antagonist.

Do not administer Virataz R tablets if both Tenofovir and an H2-receptor antagonist have to be taken together.

Proton-pump inhibitors are not to be used in treatment-experienced patients receiving Virataz R.

Do not combine Virataz R with Efavirenz in treatment-experienced patients due to decreased Atazanavir exposure.

Precautions:

HIV can be transmitted through sexual contact with HIV positive partner, blood (e.g., sharing needles, handling blood instruments, receiving blood transfusions, blood products, or organ/tissue transplants that were infected with HIV), childbirth, toiletries (razors and toothbrushes). Therefore, both HIV-positive and HIV-negative individuals are advised to have any kind of sex using a latex or polyurethane condom, avoid sharing needles or other things like toothbrushes or razors. It is important to discuss with doctor the precautions needed to avoid infecting other people as well as take precautions not to get infected.

Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.

Missing a dose

If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose of Virataz R to compensate the missed dose.

Overdose

If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store Virataz R tablets or oral powder at room temperature between 68-77 degrees F (20-25 degrees C). Keep Virataz R in the original container. Do not use Virataz R if the seal over the bottle opening is broken or missing. Keep the bottle tightly closed. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Side effects

The most common and uncommon side effects associated with Virataz R are:

  • rash
  • headache
  • dizziness
  • vomiting
  • crystals in urine
  • nausea
  • abdominal pain
  • dyspepsia
  • paresthesia
  • throat pain
  • increased liver enzymes
  • diarrhoea
  • blood in urine
  • peripheral neuropathy (tingling and numbness of feet and hand)
  • cough
  • altered taste
  • increased cholesterol level
  • increased triglyceride level
  • protein in urine

Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.

Contraindications

Virataz R is contraindicated in patients who are allergic to Atazanavir, Ritonavir or any of the other ingredients of Virataz R.

Do not use Virataz R in patients younger than 18 years.

Virataz R is contraindicated in patients with severe hepatic insufficiency, including chronic hepatitis B or C.

Virataz R with ritonavir is contraindicated in patients with moderate hepatic insufficiency.

Do not combine Virataz R with simvastatin or lovastatin, rifampicin, PDE5 inhibitors, substrates of the CYP3A4 isoform of cytochrome P450 (quetiapine, alfuzosin, astemizole, terfenadine, cisapride, pimozide, quinidine), sedative hypnotics, antiarrhythmics, ergot alkaloids (ergotamine, dihydroergotamine, ergonovine, methylergonovine), products containing St. John’s wort, atorvastatin, other anti-retrovirals (NNRTI or PI) like nevirapine or indinavir, salmeterol, hormonal contraceptives, glucocorticoids (fluticasone), voriconazole, antineoplastics (irinotecan), benzodiazepines (triazolam, oral midazolam), neuroleptics (pimozide).

Do not use Virataz R with efavirenz or proton-pump inhibitors in treatment-experienced patients.

Virataz R should be used with particular caution in association with medicinal products which can increase the QT interval and/or in patients with pre-existing risk factors (bradycardia, long congenital QT, electrolyte imbalances.

Virataz R should be used with particular caution in hemophiliac patients due to the possibility of increased bleeding.

Do not have any kind of sex without using a latex or polyurethane condom.

Do not share needles or other things like toothbrushes or razors.

Do not take Virataz R if you are pregnant or plan to have a baby without prior consulting with your healthcare provider.

Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.
