Description

Singulair® tablets, produced by Merck Sharp & Dohme, are indicated in the treatment of asthma as add-on therapy in those patients with mild to moderate persistent asthma who are inadequately controlled on inhaled corticosteroids. It helps prevent the wheezing, chest tightness, and shortness of breath caused by asthma and decreases the number of asthma attacks. The medicine is used to prevent exercise-induced bronchoconstriction (narrowing of the air passages in the lungs). It also helps treat seasonal and perennial allergic rhinitis. The active substance contained in the medication is Montelukast, a leukotriene receptor antagonist, that stops leukotrienes, chemical messengers in the immune system, from causing inflammation in the lungs and nose, preventing asthma symptoms and allergies.

Indications:

- maintenance treatment of asthma;

- prevention of exercise-induced bronchoconstriction (EIB);

- seasonal and perennial allergic rhinitis.

MOA (mechanism of action):

Montelukast is a leukotriene receptor antagonist (LTRA), used for the maintenance treatment of asthma and relief of seasonal allergies’ symptoms. Montelukast works by blocking the action of leukotrienes, which are chemicals released by the body as part of allergic and inflammatory reactions. Leukotrienes, released from the lungs in people with asthma, cause narrowing and swelling of airways in the lungs and also allergy symptoms. Montelukast blocks leukotriene receptors in the lungs and, as a result, blocks the actions of the leukotrienes. This prevents the excess mucus production, inflammation and narrowing of the airways and so prevents asthma attacks and relieves seasonal and perennial allergic rhinitis in patients exposed to substances which trigger their allergy (allergens, e.g. pollen).

Dosage

Asthma:

- Adults (15 years of age and older): 10 mg a day.

- Children (6 to 14 years of age): 5 mg a day.

- Children (2 to 5 years of age): 4 mg a day.

Singulair® should be taken once a day in the evening.

Exercise-Induced Bronchoconstriction (EIB):

- Adults (15 years of age and older): 10 mg a day.

- Children (6 to 14 years of age): 5 mg a day.

- Children (2 to 5 years of age): 4 mg a day.

For prevention of EIB, Singulair® should be taken at least 2 hours before exercise. Do not take more than one dose in 24 hours.

Allergic Rhinitis:

- Adults (15 years of age and older): 10 mg a day.

- Children (6 to 14 years of age): 5 mg a day.

- Children (2 to 5 years of age): 4 mg a day.

For allergic rhinitis, Singulair® should be taken once a day.

Patients with both asthma and allergic rhinitis should take only one Singulair® dose daily in the evening. Pediatric patients are advised to take chewable tablets.

Missing a dose

If you miss a dose of Singulair® you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a missed dose. Do not exceed the doctor’s recommended dose.

Overdose

If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Keep your tablets in the blister pack until it is time to take them. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

The most common side effects associated with Singulair® are:

Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.

Contraindications

Do not take the Singulair® if you are allergic to Montelukast or any of the other ingredients of this medicine.

Do not use the tablets to treat an acute asthma attack, including Status Asthmaticus.

Do not use the drug if you suffer from severe kidney failure.

Do not combine the medication with drugs which induce the CYP450 enzyme system (phenobarbital and rifampin), as they may decrease the effects of Montelukast.

Do not use the medication if you have a phenylketonuria, as Singulair® contains phenylalanine.

Do not take the medication tablets during pregnancy or while breastfeeding without consulting your doctor.

Avoid drinking alcohol while taking Singulair® tablets, as it may cause excessive drowsiness.

Do not give this medicine to children less than 2 years of age.
